• Payment

    We make every effort to keep down the cost of your medical care. Payment is requested at the time of your visit.

     

    Please remember that insurance companies pay fixed allowances on services and procedures. It is your responsibility to pay any deductible amount, co-insurance or any other balance not covered by your insurance company.

     

    Many insurance companies require us to collect co-payments at the time of your visit. Please be prepared to pay when you check in with the Receptionists.  Correct change is very helpful. Financing is available through Wells Fargo. Payment plan options are available for those that do not qualify for financing. We accept check, Master Card and Visa.

  • Insurance and Pre-Authorization
    The insurance plans and their rules change frequently. Please bring all of your insurance cards with you for every visit.

    If you have more than one insurance plan or if your spouse is covered, inform the staff so that we can help coordinate the benefits and save you money! Some insurance companies require us to check your coverage or obtain insurance pre-authorization for visits or procedures. Please alert us so we can assist you.

  • Third Party Liability

    We do not accept Third Party Liability. We request that you allow our office to file your medical insurance. If that is not possible then we expect payment at the time of service. Several payment options will be available to you if a payment plan is necessary.

  • Worker's Compensation

    Treatment for Worker’s compensation injury requires written verification and billing information from your employer.  Without this, you may be responsible for payment.

  • Billing Department

    Our billing department is available to answer all of your questions.  Please contact them directly at 336-272-4578 option 5. The office staff and billing staff will help you with insurance questions and problems.  We also encourage you to call your insurance company directly or go to their web site to check your benefits to make sure that you will be covered.

  • Self-Pay Patients

    If you do not have insurance, the payment is expected at the time of service. Our staff will gladly discuss payment options at the time you register for your first appointment.
