Disability forms are completed as a courtesy. There is a $15.00 fee to complete simple forms.  If the forms are extensive, there is a $25.00 fee. If you are a surgical patient, forms are completed at no cost to you for 90 days, from the date of your surgery.  There will be a fee on the 91^st day.

Disability updates are handled by completion of forms only. It is our policy not to disclose information to your carrier by telephone or fax.
